Instructions: All nominations for this award must be submitted using this form, adhere to any response word limitations, and typically be submitted to the chair of the Michigan TU Awards Committee by midnight of the nomination deadline date. Should the nomination fall outside of the normal time frame of the Awards Committees’ work, it must be sent to the chair of the Michigan TU Executive Committee. All information on this form must be completed by the nominating individual. Supplemental material may be presented in support of the nomination though selection will be based primarily upon information included on this nomination form. Nominations that fail to follow these instructions will not be given consideration by the Michigan TU Awards Committee or Executive Committee.
